You have decided to begin therapy... Congratulations! This is the place to begin! Here you will find all the intake paperwork you need to understand how I run my practice. Please ask questions, if you have any, BEFORE you sign the documents.

Download the paperwork, print it out, fill it all out and then get it back to me, preferably before your first session. If you cannot print them, let me know, I can snail mail them to you if all else fails!

Take a picture of the forms and email the pics to me, or scan them and email them.
